The city of Quincy has applied for and received state funding to provide grants of up to $5,000 to local, small businesses that have been severely harmed by the months-long effort to stop the spread of COVID-19 through self-isolation and social distancing.

APPLICATION TO THE CITY OF QUINCY’S SMALL BUSINESS EMERGENCY GRANT PROGRAM

The application packet above includes the following:

• Program Guidelines (3 pages)
• Program Application (3 pages)
• A W9 form to complete (1 page)
• A Certification Letter (5 pages)

Applications will not be accepted after December 27, 2020

Eligible businesses (per the Program Guidelines) must show significant disruption or temporary closure (business interruption) due to the COVID-19 pandemic to qualify for a grant. Business interruptions can include:

• Decrease in revenue caused by closing or limiting access to the business establishment to comply with COVID-19 prevention directives or to otherwise prevent the spread of COVID-19 within the business establishment
• Decreases in revenue caused by decreased customer demand as a result of COVID-19
• Other revenue reductions approved for reimbursement from the Coronavirus Relief Fund by the U.S. Department of the Treasury.

The Program Guidelines identify ineligible businesses and unacceptable uses of the grant.

An application is considered complete if it includes the completed three page application, the completed W9, the signed certification letter, and documentation that shows the grant funding was spent on acceptable uses from March 1, 2020 – December 30, 2020. The documentation must be clearly understandable and can include, but is not limited to, bank statements, cancelled checks, and invoices showing method of payment.

Please Note:

• The city of Quincy will only consider complete applications.
• Complete applications will be considered on a first-come, first-served basis as funding is available.
• Complete applications will be considered on a rolling basis.
• The city of Quincy reserves the right to require the recipient of an emergency grant to repay the full amount of the grant to the city if it is determined that the recipient’s grant application included false and/or fraudulent information.
